Plan administrators1 of Ontario registered plans have been required by the Financial Services Commission of Ontario (FSCO) to complete Form 7: Summary of Contributions/Revised Summary of Contributions (Form 7) for any plan with a fiscal year that commenced on or after July 1, 2000.
